To join FICAP, select one of the following application formats and return the application, with full payment, to the FICAP office.

1. Fillable Word Form - This is a word document, which allows you to save your typed information.
    

2. Fillable PDF Document - Open the file, use the "hand tool" at the top and move to the Date field. Enter the information and TAB to the next field. When you come to the check boxes, use your mouse to check the appropriate box.
   
   NOTE: This file is available in PDF Format, requiring the free Adobe Acrobat Reader. If you do not have the reader, you can down load it from the Adobe Web Site. Unless you have the full Acrobat program, you will not be able to save the file, so print additional copies for your records.

Please be sure to include your check, made payable to FICAP, in the amount of $1000 for your first years' annual dues or complete the credit card information at the bottom of the form.

All FICAP members are encouraged to join the Political Action Committee (PAC).
